Section A

With reference to your current OR previous company and its operations, perform a qualitative risk assessment for the selected workplace and activity(ies).

1. Provide a short introduction of your current or previous company, its operations, and the activities that take place.
2. The risk assessment cannot include administrative functions (i.e. office work). You should use least one activity that is more hazardous (e.g. metal fabrication, chemical handling, factory assembly line, packing and distribution, waste handling, etc.) to answer this question.
3. Perform a risk assessment for the activity described.
   • In your risk assessment, you must identify at least FOUR hazards of your choice based on the hazard categories below:
     1. Physical hazard
     2. Chemical hazard
     3. Biological hazard
     4. Ergonomic hazard
     5. Psychosocial hazard
   • Identify the ill-health / injury for the respective safety and health hazards.
   • For each of the hazards, describe in detail at least ONE existing control measure AND/OR propose at least ONE additional control measure after risk evaluation is done. You should have at least three hazards that require additional control measures.
   • Use a 5×5 risk matrix to evaluate the risks. Include the tables used for determining likelihood, severity, and risk levels.

Points to note:

• There could be more than one hazard for the activity identified.
• Indicate the type of hazard (e.g. physical, chemical, etc.) next to the hazard description.
• Cite the references use. This may include severity and likelihood ratings, risk matrix, risk action plan, source of control measures, etc.
• A sample of the risk assessment format from Workplace Safety and Health Council is attached for your use (Annex 1). You may use or adapt the format for your risk assessment.
• To allow you to explain your activity/ hazards/ control measures, you may attach photos for better illustration.

Section B

Use the risk assessment was done or activity described in Section A to answer this section.

1. Identify at least TWO legal and regulatory requirements (state the regulations title and one specific requirement from the regulations) that are relevant to the activity described. Explain the relevance of the identified legal and regulatory requirements.
2. For each of the legal and regulatory requirement identified:
   • State the regulatory body that is overseeing the provision of the cited legal and regulatory requirements.
   • Identify the documented information that can be used for the evaluation of compliance. Explain how the identified documented information helps in demonstrating legal compliance.

Section C

Use the risk assessment was done or activity described in Section A to answer this section.

1. To further mitigate the risks of the activity, you are required to come out with a format and thus, develop a Safe Work Procedure (SWP) for the selected activity.
2. Explain how you will implement and communicate the SWP to ensure safety at the workplace.

Point to note:

• Use the requirements in Clause 7 (e.g. Clause 7.4 Communication and 7.5 Documented Information) and Clause 8 (e.g. Clause 8.1.1 General and Clause 8.1.2 Eliminating hazards and reducing OH&S risks) of ISO 45001: 2018 (Annex 2) to aid you in developing and implementing the SWP.
