Objective :

Scenario :

Lara, an 8-year-old girl is admitted to the pediatric unit with an acute exacerbation of bronchial asthma. She complained of increased tightness in his chest and shortness of breath (SOB) one day prior to admission.  Her best peak expiratory flow (PEF) meter reading is 50% and her breathing has not improved even with the use of an albuterol metered-dose inhaler (MDI).

On examination, Lara is alert, speaking in short sentences due to breathlessness even at rest.  She has coarse expiratory wheezes throughout both lung fields with decreased breath sounds at the right base of the chest.  Her oxygen saturation at room air is 89%, color is ashen, and has dark circles under his eyes. She is sitting in an upright position and using respiratory accessory muscles for breathing.
